Before Your Visit....
1. If you prefer to fill out your forms in the office, please arrive 15 minutes early
for your appointment so you may do so. If you prefer to complete them
ahead of time, please download them from our "Forms" page,
fill them out, and bring them with you.
2. Think about your health goals in the short and long term. We will address both at your first visit!
3. If you take a large number of medications or supplements, consider bringing
them with you or writing down the names and dosages so you don't
have to try to remember them during the visit.
